Actually, I was figuring we'd just mandate that all the majors need to take all comers and those who can't afford it would be subsidized by the feds to keep docs, hospitals, and insurers from just shifting their costs over to us in a less efficient fashion. So, with an MSA we'd all self insure (or be subsidized to self-insure) for the first $2k (or whatever), and then we'd let the free market figure out the best way to get everyone covered equitably.
