OK, I'm going to weigh in here because I actually live in Japan. I'm a Canadian who has been living in Japan for the past 27 years.
I can tell you that Ivermectin is NOT being used to treat COVID-19 in the hospitals. It's true there is no war or misinformation campaign against Ivermectin in Japan as there is in other countries but it's still not really widely advertised or used in a hospital setting.
I did watch that YouTube video "Miracle in Japan" and I can say I really doubt the argument (around the 12 minute mark) that the Japanese have this defence enzyme called APOBEC3A which is contributing to the COVID-19 virus waning in Japan.
The reason I doubt the APOBEC3A enzyme argument is that the Japanese are genetically similar to the South Koreans. And right now while Japan is witnessing a dramatic decrease in COVID infections, hospitalizations and deaths South Korea (which has a similar fully vaccination rate as Japan) is witnessing a dramatic increase in COVID infections, hospitalizations and deaths.
There is something else going on and I don't what it is for the decrease in COVID-19 in Japan. It truly is a mystery. I also don't believe it's the vaccines either because the vaccines have NOT eliminated COVID-19 in any other country. In addition, how do you explain the dramatic spike in August and then the dramatic collapse in COVID-19 infections, hospitalizations and deaths in Japan? I mean even if you thought the vaccines were effective (which they are not) the collapse wouldn't have been so dramatic. The reduction in COVID infections, hospitalizations and deaths would've been more gradual.
Something else is going on here in Japan and no one knows. It is truly a mystery!
